Fee schedule:

• Initial medical/psychiatric evaluation (1 hour) $350

• Medication management (½ hour) $175

• Medication management and/or psychotherapy (1 hour) $350

• Family / couples therapy (1 hour) $350

• Home visits, including round trip drive time $450/hour

• Forms/letters/calls/prior authorizations

(up to 5 minutes at no charge; thereafter prorated at $350/hour)

• Court / expert witness testimony $450/hour

• Missed appointment / no show ½ price of appointment missed

Payment for services is due at the time of service.  I accept cash, check, and credit card payments.  I do not participate as a contracted provider for any insurance company, and am considered “out of network” for which some PPO/HMO insurances may partially reimburse. You may request a receipt for each visit, called a superbill, that you submit to your insurance company for reimbursement. Your insurance may not cover these fees (in part or in full).

At this time, I  am not "opted out" and cannot accept any new Medicare/Medicaid patients.  Appointments canceled less than 1 business day prior to the scheduled time will be charged for half the price of the appointment, with rare exceptions.  Patients who arrive 20 minutes late or longer to an appointment may need to reschedule and are responsible for the full cost of the appointment.
